Your first step in getting started with Concord Medical Centre is to book an appointment date. All appointments are virtual. We recommend booking a VIDEO consultation so you can meet the Concord staff face-to-face. Phone consultations are available to patients without access to internet or a video device.

Complete Intake Questionnaire
If you are booking online, once you have found an appointment day and time that works for your schedule, you will be prompted to complete the required pre-visit questionnaires. The questionnaires review your medical history, reason for seeking medical cannabis, as well as a few short standardized questionnaires that assist the doctor in their assessment. A Patient Care Assistant will share these to your phone and/or email if you are booking over the phone. Those without access to a computer/internet can complete over the phone with a Concord team member. These questionnaires must be completed prior to your scheduled appointment, otherwise your time slot may be forfeited.
Obtain Physician Referral
We can help you with this! Included in your intake questionnaire package (above) is a request for us to obtain a referral from a physician, usually your GP/Family Doctor. You may consider speaking with your doctor at your next visit with them. They can fax a completed referral form to 1-844-380-3980. Please contact the clinic if you do not have a family doctor and haven’t seen a doctor for your primary concern.
A Patient Care Assistant will contact you by phone, text and/or email to confirm your appointment. Detailed instructions will be provided for those doing a consultation by video. Expect a phone call at the scheduled time if you have booked your consultation to be by phone.
